Our home health census is growing, and we’re looking for compassionate and talented **Speech-Language Pathologists (SLPs)** to join our expanding therapy team in **St. Clair Shores, Eastpointe, Grosse Pointe, Monroe, and Belleville!**

❤️ **What You'll Do**

As a member of our home health therapy team, you’ll provide personalized, one-on-one care to help patients improve **communication, cognition, swallowing, safety, and overall quality of life** in the comfort of their homes.

**SLPs will:**

-   Complete comprehensive speech-language evaluations to assess communication, cognition, voice, and swallowing needs.
-   Develop individualized plans of care and measurable treatment goals.
-   Provide evidence-based interventions for speech, language, cognitive-communication, and swallowing disorders.
-   Assess and treat conditions including dysphagia, aphasia, dysarthria, cognitive impairment, and other communication disorders.
-   Reassess patient progress and modify treatment plans as clinically appropriate.
-   Provide skilled speech therapy services in accordance with physician orders and **CMS Conditions of Participation (CoPs)**.
-   Educate patients and caregivers on communication strategies, swallowing precautions, aspiration prevention, cognitive techniques, adaptive tools, and individualized Home Exercise Programs (HEPs).
-   Maintain timely, accurate documentation in accordance with CMS, payer, and agency requirements.
-   Collaborate with physicians, caregivers, nursing, therapy, and the interdisciplinary team to support successful patient outcomes.

## Requirements

💼 **Our Ideal HHC Guru**

You may be our next **Home Health Magic Maker** if you have:

-   Degree from an accredited **Speech-Language Pathology** program.
-   Current **Michigan Speech-Language Pathologist license** in good standing.
-   Certificate of Clinical Competence (**CCC-SLP**) preferred, as applicable.
-   Home health experience preferred.
-   Strong clinical judgment, critical-thinking, and problem-solving skills.
-   Excellent communication, interpersonal, and patient education skills.
-   Ability to work independently while remaining connected to the interdisciplinary care team.
-   Reliable transportation, valid driver's license, and current auto insurance.
-   Familiarity with Medicare/CMS documentation preferred.
