**What You’ll Do**

As a Home Health RN, you’ll bring **skilled, heartfelt care** directly to patients and families. Your days will be filled with **purpose** as you:

-   Provide one‑on‑one nursing care in the comfort of patients’ homes
-   Complete assessments, develop care plans, and deliver high‑quality clinical care
-   Educate patients and families to support independence and confidence
-   Collaborate with physicians, therapists, and your care team to ensure seamless care
-   Document visits accurately and timely using our user‑friendly EMR
-   Support patients through recovery, chronic condition management, and health maintenance

## Requirements

**What You'll Bring**

-   Active RN license
-   Active BLS certification
-   Active CPR certification
-   Experience with OASIS-based home health documentation
-   Experience with care planning, medication management, and coordination of care
-   Experience working with patients and families, physicians, therapists, and the full care team
-   Comfort with technology and electronic documentation systems
-   Eligible to work in the United States
-   Reliable transportation
-   Home health experience is a plus, but not required — we’re happy to train the right nurse
-   Bachelor of Science in Nursing (BSN) is a plus
-   Experience with Kinnser is a plus
