### 💙 You Might Be a Great Fit If You:

-   Conduct comprehensive physical therapy evaluations to assess functional needs, develop individualized treatment plans, provide evidence-based interventions, and monitor progress to maximize mobility, safety, and independence.
-   Deliver skilled physical therapy services in accordance with physician orders and the CMS Conditions of Participation (CoPs).
-   Maintain timely, accurate documentation while ensuring compliance with CMS, payer, and agency standards.
-   Collaborate with patients, caregivers, physicians, and the interdisciplinary team to optimize mobility, functional outcomes, and fall prevention.
-   Educate patients and caregivers on therapeutic exercises, gait training, transfer techniques, fall prevention, assistive devices, and individualized Home Exercise Programs (HEPs) to promote safe mobility and recovery.
-   Reassess patient progress, modify treatment plans as appropriate, and communicate changes to the care team.

## Requirements

### **💼 What We're Looking For:**

-   **Degree from an accredited Physical Therapy (PT) or Physical Therapist Assistant (PTA) program.**
-   **Current Michigan Physical Therapist/Physical Therapist Assistant license in good standing.**
-   **Home health experience preferred (minimum one year).**
-   **Strong clinical judgment, critical thinking, and problem-solving skills.**
-   **Excellent communication, interpersonal, and patient education skills.**
-   **Ability to work independently while collaborating effectively with an interdisciplinary team.**
-   **Reliable transportation, a valid driver's license, and current auto insurance.**
