**Role Overview**  
We're seeking a Part-Time Primary Care NP to provide on-site care to Assisted Living/RCFE residents across Bakersfield and Terra Bella.

**What You'll Do**

-   Routine & urgent assessments; chronic disease management
-   Post-discharge follow-ups, med reconciliation, labs/diagnostics
-   Coordinate with facility staff, specialists, families/POAs
-   Same-day documentation; CMS/regulatory compliance

**Schedule & Setting**

Part-time position — 3 days per week delivering on-site care across Bakersfield and Terra Bella with strategically clustered routes designed to minimize travel and maximize time with patients.

## Requirements

Qualifications

-   Active CA NP & RN licenses; active furnishing license; NPI, DEA, BLS
-   Geriatrics or facility-based experience preferred; EMR proficiency
