AllCare.ai grew to 2,500 seniors in 150 senior living communities across California in under two years, running on our own AI-powered care coordination platform. We're expanding to Bergen and Morris counties and hiring nurse practitioners to lead care for residents of local assisted living residences and personal care homes.

What you'll do

-   Complete initiating visits and provide proactive primary care in senior living communities
-   Enroll residents in our care management programs (CCM, APCM and PCM) and build their care plans with our coordination team
-   Work clustered routes, with telehealth where allowed

## Requirements

You're a fit if you

-   Hold an active New Jersey APN certification and national board certification (FNP or AGNP preferred)
-   Are enrolled in Medicare or able to enroll, and have (or can get) a New Jersey DEA registration
-   Have geriatric, primary care or long-term care experience (preferred)
