AllCare.ai grew to 2,500 seniors in 150 senior living communities across California in under two years, running on our own AI-powered care coordination platform. We're expanding to Long Island and hiring nurse practitioners to lead care for residents of local assisted living communities and adult homes.

What you'll do

-   Complete initiating visits and provide proactive primary care in senior living communities
-   Enroll residents in our care management programs (CCM, APCM and PCM) and build their care plans with our coordination team
-   Work clustered routes, with telehealth where allowed

## Requirements

You're a fit if you

-   Hold an active New York NP license and national board certification (FNP or AGNP preferred)
-   Are enrolled in Medicare or able to enroll, and have (or can get) a New York DEA registration
-   Have geriatric, primary care or long-term care experience (preferred)
