### **Nurse Case Manager - Clinic Setting**

### **Location: Poplar, MT**

We are seeking an experienced **Nurse Case Manager** for a 1-year contract with an ASAP start. This role focuses on coordinating patient care, managing case plans, and ensuring quality outcomes in a clinic environment.

-   Contract Length: 1 year
-   Start Date: ASAP
-   Pay: $55-$60/hr (DOE)

### **Responsibilities:**

-   Assess, plan, and coordinate patient care and case management services
-   Collaborate with providers, patients, and interdisciplinary teams
-   Ensure compliance with clinic policies, regulations, and care standards
-   Manage documentation and reporting requirements

### R**equirements:**

-   Active RN license
-   **Must have Purchased Referred Care (PRC) experience**
-   Clinic or outpatient experience preferred
