## Description

_**Position Title:**_ Crisis Program Coordinator

_**Salary:**_ $68,000 - $72,000  

_**Responsibilities:**_

-   Lead the Involuntary Outpatient Commitment and Crisis Diversion Teams, including staff supervision, scheduling, performance feedback, and on-call coverage.
-   Provide clinical supervision and professional support, including regular supervision, documentation, development, and licensure guidance.
-   Conduct comprehensive clinical assessments and ongoing evaluations to identify consumer needs and appropriate services.
-   Provide crisis and behavioral health services, including de-escalation, supportive counseling, outreach, and crisis response.
-   Collaborate with multidisciplinary teams to coordinate care, share clinical information, and ensure continuity of services.
-   Develop individualized treatment plans with consumers based on their goals, strengths, needs, and preferences.
-   Monitor consumer progress and changing needs, including symptoms, behaviors, medication compliance, daily living skills, and treatment engagement.
-   Promote consumer independence through coaching and support with daily living, life skills, and symptom management.
-   Connect consumers with essential resources, including housing, benefits, transportation, entitlements, and other community services.
-   Educate consumers and families on mental health, crisis prevention, conflict mediation, symptom management, and available supports.
-   Coordinate with community and legal partners, including screening centers, healthcare providers, Superior Court, and the Ocean County Adjuster’s Office.
-   Oversee quality and program operations, including documentation, communication, billing support, confidentiality, consumer rights, and compliance.

## Requirements

-   Master's Degree in Social Work, Psychology or related field
-   LSW/LAC Required
-   LCSW/LPC Preferred
-   Experience doing outreach with the SMI/MICA population, knowledge and awareness of community services available
-   Excellent written, verbal and interpersonal communication
-   Valid NJ Driver’s license with less than 6 points.
