Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to California’s most vulnerable individuals, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. We develop, own, and manage multi-family supportive housing and licensed residential care homes. We implement a broad array of scattered-site, one-on-one supportive housing, and clinical case management programs through partnerships with developmental services, homeless services, veterans’ services, and health care sectors.

**Program Summary**

The case management department provides intensive case management services to three supportive housing programs in Los Angeles County; Permanent Supportive Housing (PSH) Project- Based, PSH Scattered-Site, and Breaking Barriers. Our programs provide services in different housing setting, including scattered-site and project-based, serving participants that experience homelessness, have a chronic physical or mental illness, people exiting jail/prison or hospital settings.  Our team of Case Managers are dedicated to a Housing First approach that values client agency, dignity, and personal growth.

**Position Summary**

The Temporary Case Manager will provide intensive case management services to a caseload of 20-30 program participants. The Case Manager will assist clients to reach their identified goals and connect clients to community resources and services. The Case Manager will work collaboratively with necessary collaborative partners including the housing department, property management and/or employment teams to ensure that clients reach the ultimate goal of self-sufficiency. The Case Manager plays a critical role in supporting clients to thrive through a client-centered and “whatever it takes” approach to services.

**RESPONSIBILITIES**

1.  Conduct assessments and re-assessments, including a comprehensive psychosocial assessment, in order to evaluate clients’ needs and strengths.
2.  Develop and implement, in collaboration with clients, individualized case management services plans that ensure housing stability and identify short and long-term goals.
3.  Provide support throughout the housing process including but not limited to temporary housing placement, unit identification, application submissions, lease signing, and ongoing housing retention.
4.  Meet with clients frequently and regularly in the home, community, and office. Accompany clients to appointments as needed.
5.  Utilize evidence-based practices, including Cognitive Behavioral Interventions and Motivational Interviewing, in order to strategically address clients’ needs.
6.  Provide crisis intervention and ensure that clients have access to the 24/7 case management crisis line.
7.  Connect clients to a variety of community resources and services, including physical and mental health services, substance use treatment, government benefits, education/vocational programs, parenting programs, employment services, etc.
8.  Provide life skills education and assistance, including budgeting, apartment upkeep, stress management, conflict resolution, etc.
9.  Collaborate and communicate with all collaborative partners on a regular basis.
10.  Build and maintain partnerships with community organizations, service providers, public entities, and landlords to ensure that clients have access to comprehensive services.
11.  Participate in case conferences to review clients’ progress and address any challenges, with particular emphasis on case coordination and conferencing 
12.  Provide transportation assistance, including transportation via personal vehicle (depending on specific program & contractual obligations)
13.  Maintain updated and accurate client files and documentation in Salesforce, CHAMP and BOX
14.  Participate in case conferences, staff meetings, 1:1 supervision and other agency events as required

## Requirements

**Professional Experience**

-   At least 2 years of experience providing direct mental health and/or intensive case management services to vulnerable populations **OR** at least one year of experience and a Bachelor’s Degree in Social Work, Psychology, Sociology, or a related field
-   Experience working with individuals who have experienced homelessness
-   Experience working with individuals who have a mental illness, chronic health issue, and/or substance use disorder , or criminal justice system

**Knowledge, Skills and Ability**

-   Strong interpersonal and client engagement skills
-   Excellent verbal and written communication skills
-   Flexibility regarding scheduling and prioritizing of tasks
-   Willingness to travel and spend significant time in the field
-   Basic computer knowledge (MS Word and Excel required)
-   Familiarity with evidence-based interventions including Harm Reduction, Trauma Informed Care, Motivational Interviewing
-   Ability to work independently and utilize critical thinking skills in decision-making situations
-   Good organizational and record-keeping skills
-   Possession of a valid, clean California driver’s license and a personal insured vehicle are required

**Certificates, Licenses, and Registrations** 

A valid, clean CA driver’s license and a personal insured vehicle are required. 

**Travel Required**

Travel throughout Los Angeles County with approximately 75% of the time spent in the field.

**Labor Union**

This position is part of the OPEIU, LOCAL 30, AFL-CIO, bargaining unit. As a member of the OPEIU, LOCAL 30, AFL-CIO, the incumbent will be subject to the terms and conditions outlined in the collective bargaining agreement.

**Physical Requirements**

Candidates should have physical mobility for tasks such as standing, bending, stooping, kneeling, crouching, reaching, twisting, and walking on uneven surfaces. They should be capable of performing stationary tasks like sitting for up to 6 to 8 hours a day.  Additionally, candidates should be able to lift, carry, push, pull light to moderate weights up to 15 pounds safely. Requires mental acuity for analytical reasoning and document interpretation.

The salary range for this position is $25.48 - $26.50. **This position is being offered at $25.48 per hour.**
