Maintains patient medical records in Hospice.  Ensures accurate and timely filing into processing patient medical assistant.  Enters data into the Hospice computer system.

**ESSENTIAL JOB FUNCTIONS:**

-   Programs phone to Answering Service at end of workday.
-   Answers department telephones, relays messages clearly, and responds to inquiries in a timely and appropriate manner.
-   Accurately, sorts and files records and reports in the Patient Medical Record (active and discharge records).
-   Collaborates with LHCSA (HHA) partner agencies to coordinate and secure appropriate Home              Health Aide placements for current patients, ensuring timely coverage and continuity of care.
-   Timely notifies all required providers of a patient’s death, including HHA agencies, DME vendors, referral sources, and interdisciplinary team members, in accordance with established procedures.
-   Orders supplies/equipment for patients assigned.  Requests pick up of rented equipment with vendors for patients discharge as needed.  
-   Verifies that HHA invoices are accurately entered into the system and ensures all reported hours are correct prior to submission.
-   Makes copies and retrieves documents upon request.
-   Works within legal, ethical practice standards to his/her position as well as Calvary’s Code of Conduct.  Complies with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer.

**Requirements**

-    High School Diploma required. Some college preferred.
-    Minimum of one year of clerical experience in an office environment is required.
-    Competency with computer software and peripherals is required.
