**Ideal Candidate**

-   A Nurse Practitioner/Physician Assistant seeking to join the Chestnut team on a part-time or 1099 contract basis
-   8-10 hours available per week, flexible day/time
-   Interested in physical medicine and rehabilitation (PM&R)

**What You’ll Do**

-   Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations, to determine appropriate rehabilitation goals and interventions.
-   Collaborate with therapists and the facility’s medical team on treatment plans.

-   Maintain accurate and timely documentation of initial evaluations, treatment plans, progress notes, and discharge summaries in electronic medical records systems.

## Requirements

**Requirements**

-   Active Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice
-   If an NP: ANCC or AANP certification
-   Current DEA and state Controlled Substance License (required upon start date)
-   SNF, pain management, or physiatry experience a plus
