### **Community Health Services Nurse Practitioner / Physician Assistant (NP/PA)**

### **Position Summary**

The Community Health Services Nurse Practitioner or Physician Assistant provides comprehensive primary and preventive healthcare services to individuals and families within a community-based setting. This role focuses on improving access to care, managing acute and chronic conditions, promoting health equity, and delivering culturally competent, patient-centered care to diverse and underserved populations.

### **Essential Duties and Responsibilities**

-   Provide comprehensive primary care services including health assessments, diagnosis, and treatment of acute and chronic conditions
-   Deliver preventive care services such as health screenings, immunizations, and wellness education
-   Manage chronic diseases including diabetes, hypertension, asthma, and cardiovascular conditions
-   Provide care coordination and referrals to specialty, behavioral health, and social support services
-   Address social determinants of health and connect patients with community resources
-   Promote health education, disease prevention, and lifestyle modification
-   Provide care across the lifespan as appropriate to scope of practice
-   Collaborate with physicians, nurses, care managers, social workers, and community partners
-   Maintain accurate, timely clinical documentation in the electronic health record
-   Participate in population health initiatives, outreach programs, and quality improvement activities
-   Support care transitions and continuity of care
-   Comply with all regulatory, accreditation, and professional standards

### **Qualifications**

-   Graduate of an accredited Nurse Practitioner or Physician Assistant program
-   Current NP or PA certification and state licensure
-   Eligibility for credentialing and privileging
-   DEA registration or eligibility
-   Prior experience in community health, primary care, or public health preferred
