Elite Care Management- Indiana office, is seeking a dynamic **RN Floater / Case Manager** to join our team. This role is ideal for a nurse who thrives in a flexible environment, enjoys variety, and is looking to expand both **clinical and leadership skills**.

**Position Overview**

As an RN Floater / Case Manager, you will be cross-trained across multiple patient cases, providing both **hands-on skilled nursing care** and **case management oversight**. This includes supporting coverage needs while ensuring high-quality, compliant patient care.

**Key Responsibilities**

-   Perform supervisory visits and routine skilled nursing visits
-   Provide shift coverage as needed across assigned cases
-   Manage and coordinate patient care plans in collaboration with physicians and interdisciplinary team
-   Monitor patient progress and update care plans accordingly
-   Ensure compliance with all clinical, documentation, and regulatory standards
-   Serve as a clinical resource for field staff and caregivers

## Requirements

-   Active RN License (State of Indiana)
-   Current CPR Certification
-   Home health or case management experience preferred
-   Strong clinical assessment and critical thinking skills
