## Description

**RN Supervisor, Home Care Services**

 Responsibilities:

-    Completes initial comprehensive and ongoing assessment of patient and family to determine best services to meet their needs
-   Coordination of services with the patient, family, and care giver team.
-   Ongoing supervision of all professional and para-professional staff assigned to the patient.
-   Ongoing communication with patient and family to ensure client satisfaction and best outcomes

## Requirements

-   _**Registered Nurse (RN) license NYS**_
-   _**BSN required**_
-   **3 + years of clinical experience**
-   _**Home care experience**_
-   Minimum 1 year of supervisory experience
-   Excellent observation, problem solving and communication skills
-   Compassion and integrity
