### Job Description

We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide direct patient care in a primary care clinical setting. The LPN will work under the direction of physicians, registered nurses, and advanced practice providers to support patient care, perform clinical procedures, monitor patient conditions, administer medications, and promote positive patient outcomes. The ideal candidate is committed to delivering exceptional patient care while maintaining high standards of clinical quality, safety, and professionalism.

### Key Responsibilities

-   Provide direct nursing care within the scope of licensure.
-   Obtain and document patient vital signs, medical histories, and health information.
-   Administer medications, injections, immunizations, and treatments as prescribed.
-   Assist physicians, nurse practitioners, physician assistants, and registered nurses with examinations and clinical procedures.
-   Perform specimen collection, wound care, dressing changes, and other clinical treatments.
-   Educate patients on medications, treatment plans, preventive care, and healthy lifestyle practices.
-   Accurately document patient care in the Electronic Health Record (EHR).
-   Monitor patient conditions and promptly report changes to the supervising provider or registered nurse.
-   Maintain infection prevention, patient safety, and quality standards.
-   Collaborate with the healthcare team to ensure coordinated, patient-centered care.

## Requirements

### Requirements

-   Graduate of an accredited Practical Nursing program.
-   Active Licensed Practical Nurse (LPN) license in the state of practice or eligibility for licensure.
-   Current Basic Life Support (BLS) certification.
-   Current CPR certification (if not included with BLS).
-   Advanced Cardiac Life Support (ACLS) certification preferred for specialty clinical assignments.
-   Minimum of one (1) year of direct patient care experience within the past three (3) years.
-   Experience in primary care, family medicine, internal medicine, medical-surgical, emergency department, telemetry, or critical care preferred.
-   Proficiency using Electronic Health Record (EHR) systems and clinical documentation software.
-   Strong clinical, communication, organizational, and patient care skills.
-   Ability to work collaboratively within a multidisciplinary healthcare team.
-   Excellent critical thinking, time management, and interpersonal skills.
-   Must be authorized to work in the United States. **U.S. Citizens and Lawful Permanent Residents (Green Card holders) are encouraged to apply.**
-   Ability to successfully complete all pre-employment requirements, including background screening, credential verification, and drug screening, if applicable.
