-   Intake and evaluation process of patients in long-term care facilities.

## Requirements

**Qualifications:**

-   **Education:** Associate’s or Bachelor’s degree in Nursing (ADN/BSN).
-   **Licensure/Certifications:**

-   Active New York RN license.
-   MDS Assessor Certification (required).

-   **Experience:** 1–2 years in **long-term care with MDS coordination experience**.
