### Position Overview

A well-established outpatient primary care clinic in Sierra Vista, Arizona, is seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to join its collaborative care team.

In this role, you will provide comprehensive primary care to patients across the lifespan, including preventive services, acute care, chronic disease management, wellness visits, and patient education. You will have the clinical autonomy to manage your patients while benefiting from an experienced team of physicians, providers, care coordinators, and clinical support staff.

### Key Responsibilities

-   Provide comprehensive outpatient primary care to patients of all ages
-   Conduct physical examinations and obtain complete medical histories
-   Evaluate, diagnose, and treat acute illnesses and common medical conditions
-   Manage chronic conditions such as diabetes, hypertension, asthma, and hyperlipidemia
-   Perform annual physicals, wellness visits, preventive screenings, and follow-up appointments
-   Order, interpret, and follow up on laboratory tests, imaging, and other diagnostic studies
-   Develop individualized, evidence-based treatment plans aligned with patient needs and goals
-   Prescribe medications and monitor treatment effectiveness
-   Educate patients and families about disease prevention, chronic condition management, medications, and healthy lifestyle choices
-   Coordinate referrals and collaborate with physicians, care coordinators, and clinical support staff
-   Maintain accurate and timely documentation in the electronic health record
-   Follow current clinical guidelines, regulatory requirements, and standards of care
-   Participate in continuing education and quality-improvement initiatives

### Minimum Qualifications

-   Graduate of an accredited Nurse Practitioner or Physician Assistant program
-   Active and unrestricted Arizona NP or PA license
-   Current national board certification appropriate to the provider’s discipline
-   Valid, unrestricted DEA registration
-   Eligible for credentialing with Medicare, Medicaid, managed care organizations, and commercial insurance plans
-   Strong clinical judgment, communication, and organizational skills
-   Commitment to compassionate, patient-centered care

### Experience

-   Experienced primary care providers are encouraged to apply
