**Schedule:** Monday–Friday | No Weekends | No On-Call

### **Position Overview**

We are seeking a Nurse Practitioner or Physician Assistant to provide full-spectrum outpatient primary care to patients across all ages. This role emphasizes continuity of care, patient education, and improving health outcomes within our community.

### **Key Responsibilities**

-   Conduct comprehensive physical exams and review diagnostic results
-   Diagnose and treat acute illnesses (e.g., infections, minor injuries)
-   Manage chronic conditions including diabetes, hypertension, heart disease, and obesity
-   Develop and implement individualized treatment plans
-   Provide patient-centered care addressing physical, emotional, and social needs
-   Collaborate with physicians, nurses, and specialists for coordinated care
-   Educate patients and families on conditions, treatment options, and wellness strategies
-   Coordinate referrals, follow-ups, and hospital admissions when necessary
-   Maintain accurate, timely documentation in the EMR
-   Lead wellness screenings and preventive health initiatives
-   Maintain current clinical knowledge through continuing education

### **Required Qualifications**

-   Active Nurse Practitioner or Physician Assistant license in Arizona (or eligibility in progress)
-   Graduate of an accredited U.S.-based NP or PA program _(Puerto Rico programs not accepted)_
-   **Current or eligible DEA registration**
-   **BLS certification required; ACLS preferred**
-   **Minimum 1 year of primary care experience preferred**
