### Position Overview

A community-focused primary care clinic in Casa Grande, Arizona, is seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to join its outpatient family medicine team.

In this role, you will provide comprehensive primary care to pediatric, adult, and geriatric patients. Your practice will include preventive services, acute care, chronic disease management, wellness visits, and patient education.

### Key Responsibilities

-   Provide comprehensive outpatient primary care to patients of all ages
-   Conduct physical examinations and obtain complete medical histories
-   Evaluate, diagnose, and treat acute illnesses and common medical conditions
-   Manage chronic conditions such as diabetes, hypertension, asthma, and hyperlipidemia
-   Perform annual physicals, wellness visits, preventive screenings, and follow-up appointments
-   Order and interpret laboratory tests, imaging, and other diagnostic studies
-   Develop individualized, evidence-based treatment plans
-   Prescribe medications and monitor treatment effectiveness
-   Educate patients about preventive care, healthy lifestyle choices, and medication adherence
-   Coordinate referrals and follow-up care with specialists and community resources
-   Collaborate with physicians, advanced practice providers, nurses, and administrative staff
-   Maintain timely and accurate documentation in the electronic health record
-   Follow all clinical, ethical, regulatory, and organizational standards

### Required Qualifications

-   Graduate of an accredited U.S.-based Nurse Practitioner or Physician Assistant program
-   Active and unrestricted Arizona NP or PA license
-   Current national board certification appropriate to the provider’s discipline
-   Current unrestricted DEA registration
-   Eligibility for credentialing with Medicare, Medicaid, managed care organizations, and commercial insurance plans
-   Strong communication, organization, and clinical decision-making skills

### Experience

-   Qualified new graduate Nurse Practitioners and Physician Assistants are encouraged to apply
