**Overview**

**We offer $1500.00 sign-on bonus, paid after 90 days of successful employment.**

We are seeking a dedicated and compassionate **Registered Nurse (RN)** to conduct in-home supervisory visits for EPSDT and VA clients. This role focuses on oversight, assessment, and support rather than skilled nursing tasks.

The RN will manage a caseload of approximately **50–70 clients**, ensuring required **face-to-face visits every 60 days** in the client’s home. This position requires strong organizational skills, independent work capability, and a commitment to high-quality, patient-centered care.

**Service Areas & Current Census (subject to change)**

**Cuba, NM** – 6 clients

**Santa Fe, NM** – 33 clients

**Las Vegas, NM** – 29 clients  

**Key Responsibilities:** 

-   Conduct in-home supervisory visits with EPSDT and VA clients every 60 days

-   Ensure compliance with all federal, state, and organizational regulations and standards

-   Assess client well-being, safety, and overall care needs

-   Document visit findings accurately and in a timely manner
-   Communicate concerns, observations, and recommendations to appropriate team members
-   Collaborate with caregivers and interdisciplinary team members to ensure coordinated care
-   Maintain complete, accurate, and up-to-date client records
-   Provide education, guidance, and resources to clients and their families
-   Adhere to all company policies, procedures, and confidentiality requirements

**Qualifications:** 

-   Current, active **Registered Nurse (RN) license in New Mexico**
-   Minimum of **1 year of nursing experience** (home health or community-based care preferred)
-   Strong assessment, documentation, and organizational skills
-   Excellent communication and interpersonal abilities
-   Ability to work independently and manage a flexible schedule
-   Demonstrated commitment to compassionate, patient-centered care
-   Valid driver’s license and reliable transportation
-   Must live in New Mexico
