### Overview

Heritage Home Healthcare is seeking a compassionate and dependable **PRN Caregiver** to provide delegated personal care services to **children and adolescents** in a home-based setting in the **Albuquerque, NM area**. In this role, you will support clients with daily living activities, supervision, and routine care, ensuring comfort, safety, and dignity in the home environment. This flexible, on-call position is ideal for a reliable caregiver who is patient, observant, and passionate about supporting youth and families.

### Key Responsibilities

-   Provide in-home supervision and support for children and adolescents
-   Assist with personal care tasks including bathing, dressing, grooming, toileting, and eating
-   Follow individualized care plans and delegated care instructions from licensed professionals
-   Support medication reminders as delegated by care plan and clinical team
-   Provide mobility assistance and support with safe positioning as needed
-   Maintain a clean, safe, and structured home environment
-   Offer emotional support, companionship, and age-appropriate engagement
-   Support daily routines, behavioral structure, and consistency in care
-   Escort or transport clients to appointments or activities as needed
-   Observe, document, and report changes in behavior, mood, or condition to the clinical team

### Qualifications

-   Must be at least 18 years of age
-   Previous caregiving, childcare, or youth support experience preferred

-   Strong observational and communication skills
-   Valid driver’s license and reliable transportation preferred
-   Must live in Albuquerque, NM or surrounding areas
