## Description

We are currently seeking a dedicated and passionate Clinical Liaison Registered Nurse (RN) to join our team at KeystoneCare. This vital role involves acting as an advocate for patients within an acute care setting until they are safely discharged to their homes. The Clinical Liaison RN is responsible for generating referrals for home health, palliative care, hospice, waiver, and private duty services by building strong relationships with case managers and social workers. In addition to meeting with patients and their families, you will ensure a seamless transition of care throughout the discharge process, guiding them through their options and verifying eligibility for care.

**Primary Job Responsibilities:**

## Requirements

**Job Requirements:**

-   Valid RN license in the state of Pennsylvania.
-   3+ years of experience in a clinical care coordinator, liaison, or case manager role preferred.
-   Self-motivated with strong time management and communication skills.
-   Clinical knowledge of disease entities common to Home Health and Hospice.
-   Valid Driver's License and a reliable vehicle are required.
-   Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
