Liberty Community Program is a leading organization dedicated to providing compassionate care and support for individuals with autism and developmental disabilities. We are currently seeking a skilled Nurse Practitioner to join our dynamic team. The Nurse Practitioner (NP) will play a pivotal role in assessing, diagnosing, and managing the healthcare needs of our clients. This position is essential for bridging the gap between patient needs and clinical services in a supportive and collaborative environment.

The Nurse Practitioner will conduct comprehensive health assessments, develop treatment plans, and provide ongoing monitoring of chronic and acute conditions. The ideal candidate should possess a deep understanding of behavioral health and be passionate about improving the quality of life for those we serve.

**Key Responsibilities:**

\- Perform thorough physical examinations and health assessments for participants.

\- Develop and implement individualized care plans based on assessment findings and client needs.

\- Diagnose and manage acute and chronic health conditions.

\- Prescribe medications and monitor their effects.

\- Provide education and support to clients and families regarding health management and wellness strategies.

\- Collaborate with a multidisciplinary team including physicians, therapists, and direct support staff to ensure coordinated care.

\- Maintain accurate and timely documentation of patient interactions and care plans.

\- Conduct follow-up visits to assess progress and adjust care plans as necessary.

## Requirements

**\- Master’s degree in Nursing or Doctor of Nursing Practice (DNP)**

**\- Valid Nurse Practitioner (NP) certification and licensure in the state of West Virginia.**

\- Experience in a healthcare setting, with a focus on behavioral health or developmental disabilities preferred.

\- Strong clinical assessment skills and ability to make independent medical decisions.

\- Excellent communication and interpersonal skills.

\- Familiarity with electronic health record (EHR) systems.

\- Ability to work collaboratively with a multidisciplinary team.

\- Valid CPR/ BLS certification.

\- Commitment to providing high-quality care and advocacy for individuals with disabilities.

\- Must have reliable transportation and the ability to travel for services as needed.

\- **Preferably familiar with drug addiction and child services.**
