**About the Role**

We're looking for an experienced Registered Nurse to provide clinical leadership and hands-on nursing care to NDIS participants with complex health needs. Working on the ground alongside our support worker teams, you'll get to know the people you support and play a genuine part in improving their quality of life.

This role suits a nurse who wants more autonomy in how they work, real relationships with participants, and the satisfaction of making a tangible difference every day.

**What You'll Do**

•     Conduct comprehensive health assessments and develop individualised care plans

•     Train staff in and oversee medication management  

•     Provide clinical supervision and support to support workers and the wider team

•     Monitor participants' health status and report changes to the medical team

•     Support the onboarding of new participants and review care plans as needs change

•     Identify, investigate and manage clinical incidents in line with policy and do NDIS reporting where required

•     Liaise with participants' medical teams, families and other stakeholders

•     Travel to SIL, SDA and participant homes to deliver hands-on care

•     Maintain accurate, timely clinical documentation and NDIS reporting

## Requirements

•     Nursing qualification with current AHPRA registration as a Registered Nurse

•     Sound understanding of NDIS requirements and managing complex health needs

•     Excellent communication skills and the ability to build strong relationships with participants, families and team members

•     Current First Aid and CPR certification

•     Valid driver's licence and access to a reliable vehicle

•     Yellow Card, Blue Card and Police Clearance

•     Flexibility to work hours that meet participant needs
