## Description

**Social Worker, Occupational Health**

This role supports patients navigating workplace injury, return to work planning, and connection to community and behavioral health resources.

**Position Responsibilities:**

• Conduct psychosocial assessments for patients seen through occupational health services

• Support return to work planning in coordination with providers, employers, and case managers

• Connect patients to community resources, behavioral health services, and financial or social supports as needed

• Assist with care coordination for complex or high risk occupational injury cases

• Document assessments and interventions in the EMR in line with clinic and regulatory standards

• Serve as a liaison between patients, providers, and employer or insurance contacts when psychosocial factors affect recovery or return to work

## Requirements

_**• Master's degree in Social Work (MSW) required; LCSW preferred - LMSW considered.**_

_**• Experience in occupational health, workers compensation, disability, or a related medical setting preferred**_

• Strong assessment and care coordination skills

• Comfortable collaborating with medical providers and employer partners

• Excellent communication and documentation skills
