This position is ideal for an experienced ECM professional who thrives in both field-based member support and team development.

### Why This Role Matters

The ECM LCM Team Lead serves as a bridge between frontline staff and leadership, helping ensure operational consistency, accountability, and compassionate member care. In addition to managing a reduced caseload, this role provides hands-on support to fellow LCMs, assists with onboarding and coaching, and helps maintain department workflows and quality standards.

This role combines direct member impact with operational leadership and is ideal for someone passionate about mentorship, problem-solving, and improving outcomes for vulnerable populations.

- Coordinate medical, behavioral health, and community-based services to support member stability and care continuity

- Build strong, supportive relationships with members through compassionate, person-centered engagement

### Outreach Support
