Registered Nurse (RN) - Hospice Case Manager

**Location:** Palm Harbor, FL (Pinellas County)

**Pay Rate:** $45.00 - $48.00 / hour

**Job Type:** Full-time, Temporary / Contract (7-Week Backfill Contract)

**Schedule:** Day Shift | Monday – Friday, 8:00 AM – 5:00 PM (40 hours/week) | **No On-Call & No Weekend Visits**

Job Summary

We are seeking an experienced **Registered Nurse (RN) Hospice Case Manager** for a 7-week backfill contract in Palm Harbor, FL. Under physician orders, you will be responsible for identifying, coordinating, and managing the total care needs for terminally ill patients and their families. Utilizing advanced assessment, intervention, and teaching skills, you will maximize patient comfort and enhance quality of life across home care and facility environments.

Key Responsibilities

-   **Care Coordination & Planning:** Serve as the primary RN Case Manager to assess physical, psychosocial, and spiritual needs. Develop, implement, and update individualized Plans of Care in collaboration with the patient, family, attending physician, and Interdisciplinary Team (IDT).
-   **Direct Clinical Care:** Perform age-specific clinical assessments, manage pain and symptoms, and safely administer, monitor, and document prescribed medications.
-   **Supervision & Delegation:** Direct and supervise care delivered by LPNs and Home Health Aides, conducting required bi-weekly supervisory evaluations and on-site visits.
-   **Interdisciplinary Collaboration:** Prepare for and actively participate in IDT care planning meetings. Integrate all disciplines to fulfill patient and family goals.
-   **Post-Mortem Support:** Assist with compassionate closure contact and support following a patient’s death in collaboration with the IDT.
-   **Documentation & Productivity:** Maintain compliance with state/federal regulations, Joint Commission standards, and organizational policies using **HomeCare HomeBase** (minimum 25 productivity points/week).
-   **Community & Safety Advocacy:** Educate caregivers and facility partners on hospice philosophy; strictly maintain infection control, Standard Precautions, and personal safety standards.

Work Environment & Expectations

-   **Territory:** Field-based assignments throughout Pinellas County based on zip codes within a designated team territory (Home or Facility setting).
-   **Caseload & Visits:** Average caseload of 17–20 patients; minimum requirement of **20 patient visits per week**.

## Requirements

Qualifications & Requirements

### Required:

-   **Education:** Associate’s Degree in Nursing (A.D.N.) or equivalent from an accredited college/technical school.
-   **Experience:** Minimum of 1 year of RN experience with **prior Hospice experience in both Patient Home and Facility settings**.
-   **Licensure & Certifications:**

-   Active, unencumbered **Florida Registered Nurse (RN) License**.
-   Valid **Florida Driver’s License** and proof of valid automobile insurance.
-   Current **BLS Certification** (American Heart Association or American Red Cross).

-   **Transportation:** Reliable vehicle to perform daily field visits across the designated territory.

### Preferred:

-   1+ years of acute or post-acute clinical nursing care.
-   Experience with specialized care for terminal and life-limiting illnesses.
-   Basic computer literacy and typing speed of 25+ WPM.
-   Experience with **HomeCare HomeBase** EMR software.
