In Toowoomba you’ll join Knight Healthcare, led by Ryan Knight. You’ll work alongside \[physiotherapists, exercise physiologists, OTs, dietitians — who coordinate care together every day. When a client’s recovery stalls because of housing, family breakdown, financial stress or isolation, you’re the person the team turns to. Your work sits at the centre of the care plan. **Your Week** Your caseload covers DVA, NDIS, Home Care Packages, workers’ compensation and private clients. One morning you might run a psychosocial assessment for a veteran adjusting to life after service. That afternoon you’re sitting down with an older client and their family to work through care options under their Home Care Package. In between, you’re talking with the physio about a shared client whose pain management keeps getting derailed by things happening at home. Day to day, you’ll be: - Completing psychosocial assessments and developing goal focused intervention plans - Providing counselling and therapeutic support to individuals and families - Supporting clients to navigate DVA, NDIS, aged care and community services - Advocating for clients and connecting them with housing, financial, legal and community supports - Working in genuine multidisciplinary care with the clinic team, GPs and external providers - Writing clear, timely reports for funding bodies and referrers - Keeping your clinical documentation in line with AASW standards and Proactive Health’s governance framework Toowoomba gives you a regional lifestyle without the remoteness. Ryan has been able to build a great ## Requirements - A Bachelor or Master of Social Work accredited by the AASW - Eligibility for AASW membership - Current professional indemnity insurance, or willingness to obtain it - A current driver’s licence and reliable vehicle - A National Police Check and NDIS Worker Screening Check, current or willing to obtain - A Queensland Blue Card, current or willing to obtain - Strong assessment, case management and communication skills - The ability to work independently in the community while contributing to a collaborative team Nice to have, but not essential: Accredited Mental Health Social Worker (AMHSW) status, experience with DVA or veteran clients, NDIS or aged care experience, and a background in trauma informed practice.
