**What would you do in this role?**

This role is responsible for managing and processing disability forms and medical records requests in a timely, HIPAA-compliant manner, ensuring accuracy and completeness. It serves as a central point of coordination between patients, physicians, attorneys, and external partners, supporting communication and legal or administrative needs. Responsibilities also include invoicing and fee collection while maintaining detailed documentation within the electronic medical record system. Additionally, the role supports document control by scanning, indexing, and auditing patient records to ensure quality and compliance.

_A Day in the Life_

-   Complete and document patient disability forms on behalf of the physicians within 5 days of receipt.
-   Process and log medical records requests from various sources (e.g. attorneys, insurance companies, patients) according to HIPAA regulations within 5 days of receipt.
-   Invoice and collect fees associated with disability forms and records requests.
-   Communicate effectively via phone with patients and document information in patients’ electronic charts;
-   Communicate as needed with patients, physicians, and staff.
-   Coordinate with external entities, including medical records companies, to complete medical records and disability forms requests as needed.
-   Coordinate with attorney’s offices as needed for depositions and other legal requirements.

_Document Control_

-   Scan documents/records into OnBase and index/file into our electronic medical records system;
-   Audit patient records for completion and follow up on errors as needed.

## Requirements

-   Certifications/Education

-   High School Diploma/GED;

-   Experience:

-   One year of experience performing customer service duties;
-   Communicating effectively and professionally with various levels of employees, outside entities and customers;
-   Reviewing work and checking for errors to ensure accuracy;
-   Using computer systems and applications, including entering data and typing.

**Preferred Qualifications - How can you set yourself apart from other applicants?**

-   Two years of related experience, or equivalent combination of education and experience in a medical setting;
-   Experience using Epic (electronic medical record system);
-   Knowledge of medical terminology.
