### **About the Role**

We are seeking a dedicated **Referral Coordinator & Patient Care Coordinator** with clinical knowledge to oversee our medical referrals, lab tracking, and patient follow-up workflows.

This role is centered on managing patient lab results and specialist referrals, communicating clinical updates directly to patients, and ensuring smooth continuity of care. The daily phone volume for lab follow-ups is manageable (approximately 30 calls per day), making this an ideal position for a detail-oriented professional who values quality patient interaction and thorough follow-through.

Our patient population consists primarily of middle-aged to geriatric adults (average age ~50 to 90+), so an empathetic, patient tone over the phone is essential.

## Requirements

### **Key Responsibilities**

-   **Lab Management & Patient Outreach:** Review lab results, contact patients directly (approx. 30 calls/day) to provide lab updates, answer baseline questions, and record detailed notes in eCW.
-   **Referral Coordination:** Process inbound and outbound specialist referrals, obtain pre-authorizations when necessary, track statuses, and ensure care continuity.
-   **Patient Communication:** Serve as a clear, empathetic primary point of contact for patients, especially older adults requiring clear instructions and active listening.
-   **Documentation & EMR Updates:** Maintain meticulous, timely patient charts and task queues within eClinicalWorks.
-   **System Operations:** Utilize Intermedia VOIP to place and receive remote calls following practice protocols.

### Qualifications & Requirements

-   **Clinical Knowledge:** Strong understanding of medical terminology, basic laboratory panels/results, and general clinical workflows (Nursing, Allied Health, or Pre-Med background preferred).
-   **Communication Skills:** Excellent, fluent spoken and written English with a clear, highly intelligible voice and minimal accent—critical for clear communication with senior patients.
-   **Experience:** Prior hands-on experience in both **Referral Coordination** and **Patient Care Coordination** within a US healthcare setup.
-   **Tech Proficiency:** Hands-on experience with **eClinicalWorks (eCW)** and web-based VOIP phone systems (Intermedia or similar).
-   **Work Setup:** Dedicated, quiet home office setup with a high-speed wired internet connection, computer equipment meeting security standards, and a noise-canceling headset.
