Nurse Practitioner – Telehealth / Home Health

**Location:** Brooklyn, NY

### Position Overview

We are seeking a **motivated, energetic Nurse Practitioner (NP)** to join a growing Home Health Agency and help build a new telehealth program from the ground up.

This is not a traditional NP position. We are looking for someone who is a **go-getter, takes initiative, enjoys building programs, and wants the opportunity to grow into a leadership role** as the program expands.

You will be the **first Nurse Practitioner dedicated to this initiative**, giving you the opportunity to help develop workflows, improve processes, establish best practices, and play an integral role in shaping the future of the program.

The ideal candidate has experience in **home health/home-based care and telehealth**, understands the needs of medically complex and homebound patients, and is comfortable working independently while collaborating with physicians, nurses, care managers, and agency leadership.

### Qualifications

-   Current and unrestricted **New York State Nurse Practitioner license**.
-   **Must be enrolled in PECOS.**
-   Previous **Home Health Agency (HHA), home care, home-based care, or similar experience** strongly preferred.
-   Prior **telehealth/telemedicine experience** strongly preferred.
-   Medicare/Medicaid experience preferred.
-   Strong clinical assessment and decision-making skills.
-   Comfortable utilizing EMR and telehealth technology.
-   Excellent communication and documentation skills.

## Requirements

### Key Responsibilities

-   Conduct telehealth assessments, follow-up visits, and ongoing clinical management of patients.
-   Evaluate patient conditions, review medical histories, medications, symptoms, and care needs.
-   Develop and update appropriate treatment and care plans.
-   Coordinate closely with physicians, home health nurses, therapists, care managers, and other members of the interdisciplinary team.
-   Identify changes in condition and help prevent unnecessary emergency department visits and hospitalizations.
-   Maintain accurate, timely, and compliant clinical documentation.
-   Support continuity of care following hospitalizations and other transitions of care.
-   Help establish **telehealth workflows, protocols, and processes** as the program develops.
-   Identify opportunities to improve patient outcomes, efficiency, and program growth.
-   Work closely with leadership to help expand the service as patient volume increases.
-   Maintain compliance with Medicare, Medicaid, HIPAA, and applicable regulatory requirements.
